To the best of my knowledge and belief, I verify that all of the information provided by me in connection with my application for employment, including in this application, and all attachments and resumes is TRUE, CORRECT, and COMPLETE. I understand that if I knowingly submit false, misleading, incomplete or omitted information, it WILL result in REJECTION of my application or TERMINATION of employment if I am hired by the Company, regardless of when discovered.

I authorize the Company and its agents to verify the information on this application, attachments and resumes and to conduct an investigation regarding my suitability for employment. If I am extended a conditional employment offer by the Company, I may be requested to submit to a job-related medical examination. The results of the medical examination will be communicated to the Company and used in determining my ability to perform the essential functions of the job, with or without reasonable accommodation. If I refuse to have the medical exam or to release the results of that exam to the Company, I will not be considered for further employment.
